This is a mandatory notice for all practitioners in accordance with Texas House Bill 4224 (Section 181.105 of the Texas Health and Safety Code)

1. HOW TO OBTAIN YOUR MEDICAL RECORDS IN OUR OFFICE

You can request your medical records by one of these way:

  • Call (214) 970-7643

  • Email [email protected]

  • Mail to 4965 Preston Park Blvd., Suite 700, Plano TX 75093

  • Hand deliver your request at your next appointment

  • Sign a release at your next appointment

Include the following in your request:

  • Full name

  • Date of birth

  • Phone number

  • Exact records you would like released, including dates

2. HOW TO CONTACT OUR LICENSING AND DISCIPLINARY AUTHORITY

For questions regarding professional chiropractic standards, licensing, or rules, you may contact The Texas Board of Chiropractic Examiners (TBCE) directly

3. HOW TO FILE A CONSUMER COMPLAINT

You can file a complaint two ways:

Filing a Complaint with the Texas Board of Chiropractic Examiners (TBCE)

Filing a Complaint with the Texas Attorney General Consumer Protection Division
